Hi Dr. Naderi,

Its nice to see your back on the boards. I'm sure your super busy with your patients, so I would like to thak you for taking some of your extra time to answer my question. My question involves a revision touch up of the infra tip lobule and excession of a slight fullness and assymetry on my right lateral side wall. This will be done in the office under local anestesia. My concern is that is that I have a little fullness in the infra tip lobule area which fives the appearance of a longer nose from the 3/4 view, and some extra lenght from the front. Will removing some of this fullness by bringing it closer to the septum make the nose appear a bit shorter? Thru a closed approach, what different apporaches are used to correct a hanging infratip lobule? One doctor told me that he would remove some of the mass (dont know the technical tern for this mass or fullness) between the infratip lobule and the septum, which would alow the area to scar upwards, while another doctor said he would do a tongue and groove procedure.
I was also wondering if you are familiar with Dr. Anil Shah and Dr. Vladimir Grigoryants and there work?

Hi and thanks. I try to get to here and answer as many questions as time allows.

I have heard of both of those docs and both seem to be good docs. Dont know them personally. I dont think they are in the same "class" as Dean Toriumi but they have good reputations.

The correction of a hanging infra-tip lobule depends on the cause of the hanging infratip lobule. If the issue is a long caudal septum then that needs to be treated. If its long buckled medial crura of the lower lateral cartilages, then those need to be treated. If it is excess soft tissue or scar tissue, those need to be treated. If the issue is extra tip grafts or improper tip sutures then those need treatment. And if the infratip LOOKS long because the alar margins (nostrils) are retracted then those need to be brought down. Tongue in groove is one of the many tools in this surgery.
