Hi Dr. Naderi,

I am having a in office rhino tweak in 2 weeks. Here is my question. At my last appt. with my previous surgeon 2 1/2 weeks ago, he gave me a steroid injection in an area of fullness on my right lateral side wall. I will be 4 and a half weeks post steroid injection when I have my in office rhino tweak. Will this cause a problem? from everything I have read, it seems that a majority of the steroid injection takes affect in the first 2-3 weeks. I have also read several articles by several reknown surgeons who say they treat soft tissue polly beak deformities once every 4 weeks until it is remedied, or surgery is indicated. Do you think at 4 and a half weeks, that the steroid should pretty much not be active anymore, and it would be safe for my revision doctor to excise in this area if necesary? Your feedback would be greatly appreciated. Thanks,

Steroids do show their effect in the first few weeks. I am not sure what you mean by a "tweak." One surgeon's tweak is another surgeons overhaul! As a rule of thumb I do not like 'in office rhinoplasty" because rhinoplasty is a precise surgery that comes down to millimeters and fractions thereof and it takes excellent setting, lighting, equipment, assistant, etc to get the best controlled results and "in office" tweaks are often not that precise and are often short cuts in order to save on hospital fees. That is just my opinion. It sounds like your surgeon is planning on removing supratip scar tissue? If you are responding to the steroids then continuing with steroid injections is the way to go as you have correctly read. As far as the steroids 4 weeks ago, you should be fine. As far as the fruitfulness of a in office tweak, I cannot make any assessments without seeing your nose and knowing what is planned. Be cautious.
